BACKGROUND: Positive mental health includes not only the absence of mental disorders but also the presence of subjective well-being, good coping strategies for life stress, and strategies for adapting to community life. It is well known that the COVID-19 pandemic has challenged mental health in general population worldwide. However, research has not measured protective factors for mental health in the general population after the declared end of pandemic by the World Health Organization. METHODS: This observational, cross-sectional study surveyed 591 Paraguayan participants aged ⩾18 years, who were recruited through an online survey. Demographic characteristics, socioeconomic status were collected as well as mental health and validated tools for hope, resilience, subjective happiness were administered. RESULTS: Of the participants, 81.6% were women, 54% were married or in a relationship and 90.7% reported an university education. The main source of stress was economic issues (30.3%). A total of 22.7% had been previously diagnosed with a mental disorder, 22.2% had consulted a mental health professional and 10.8% had consumed prescription drugs. 42.6% reported flourishing, 36.2% reported moderate and 21.2% reported languishing mental health. CONCLUSIONS: This large survey has shown that most of participants reported a flourishing mental health with high ratings at hope, resilience, and subjective happiness scales. Also, the main sources of stress were economic issues, not consequently related to the pandemic. This may add evidences to the international debate on the long term effects of the global pandemic and probably suggests that recovery processes have been collectively adopted in Paraguay.

Dermatitis artefacta (DA) is a psycho-dermatologic condition based on patients' behavioral patterns, characterized by an intentional production of cutaneous lesions on their own skin. The clinical presentation can be highly variable. Patients with DA seldom seek psychological support or psychiatric consultation. More often, they seek help from their primary care physician or dermatologist. This review article aims to provide a practical guide for the diagnosis and management of AD and affected patients. A broad literature search was performed using the PubMed and Google Scholar electronic online databases, using key words "dermatitis artefacta", "diagnosis", "management", and "psychodermatology". The search was limited to English and Spanish language articles and was supplemented with themed books and book chapters. DA can occur in a variety of clinical presentations, and physicians should suspect DA in patients with a history of psychiatric disorders or extensive use of healthcare services. The ultimate goal of DA treatment may be a proper referral to mental health services. However, the prognosis is poor even when successful mental health referrals are achieved, with low recovery rates. A useful approach may include the suggestion that a mental health provider can help with the anxiety and the distress generated by the lesions: in this case in this case it will be crucial to discuss this with the mental health provider after obtaining informed consent from the patient. Considering the difficulty in promoting patients' adherence to treatment, the ideal setting for DA treatment is a psycho-dermatologic clinic, where both dermatologic and psychological interventions can be seamlessly integrated.

Stigma and discrimination are a major ongoing problem in the field of mental health as these impact on patient outcomes, access to and acceptability of therapeutic interventions, their quality of life, general wellbeing, social inclusion and opportunities. Social stereotypes, culture and prejudices all contribute to continuing discrimination in mental health. Different settings where people function may also be sources of discrimination such as work and educational environments. The lack of knowledge and understanding of mental health/illness by individuals, their families, carers and policymakers as well as the social media reporting also impact on social attitudes to discrimination. It has been also described a relevant impact of stigma among specific social minorities reporting poor mental health such as elderly people, youths, sexual variants, persons with disability. Educational programs, raising awareness trainings and proper public policies may be developed in order to reduce stigma at social level with favourable outcomes for people with mental illness.

BACKGROUND: The number of suicides has been increasing worldwide, year after year, becoming the fourth leading cause of death among young people between 15 and 29 years of age. AIM: In this study, we explored the frequency and characteristics of suicides among the adult general population in Paraguay between 2004 and 2022, considering that suicide attempts and suicidal risk/ideation are frequent and relevant issues in the consultation activity, even if epidemiological evidence on the national rates of suicide is scarce. METHODS: In this observational, descriptive, and exploratory study, official records of all deaths by suicide were reviewed and information analyzed. In addition, an attempt was made to predict the number of suicides in the next 5 years according to a mathematical modeling. RESULTS: In the 18-year period, 5,527 suicides of adults were recorded. Patients' mean age was 36.8 ± 17 years old. A 76.77% of them were males, 77.44% were from an urban area and 25.98% from the Greater Asunción and Central Department of Paraguay. The most frequently used method of suicide was intentional self-inflicted injury by hanging, strangulation, or suffocation (all 67.6%). The expected number of national suicides in the following years from 2023 to 2027 will range between 462 and 530. Limitations include the lack of information regarding diagnoses and personal history in the suicide reports as well as the possibility of underreporting of national suicide cases. CONCLUSION: Our results represent the first large national epidemiological report of suicides in Paraguay and may be of interest for mental health professionals and health authorities in order to reduce the suicide mortality rate within the country.

The COVID-19 pandemic has had a heavy impact on daily life, leading to physical and psychosocial consequences. Nowadays, clinicians and health researchers are particularly interested in describing and facing the long-term effects of COVID-19, also known as "long-COVID syndrome". Pandemic fatigue has been defined as a cluster of demotivation, tiredness, and psychological effects that emerge gradually over time after the infection or through the adoption of the recommended measures to combat it. In this study, we report the findings of a large survey conducted in South America involving 1448 participants (mean age: 33.9 ± 11.2 years old) from Argentina, Bolivia, Uruguay, Peru, and Paraguay. An online survey was launched through the common social media based on a specific assessment aimed to detect the prevalence of pandemic fatigue and associated factors. Socio-demographic characteristics, medical, and personal information were collected; the Pandemic Fatigue Scale (PFS) and the Coronavirus Anxiety Scale (CAS) were also administered. We found mid-levels of pandemic fatigue among respondents (21.7 ± 7.95 score at PFS) as well as significant anxiety related to the COVID-19 pandemic (1.56 ± 2.76 score at CAS). In addition, pandemic fatigue was significantly associated with the experience of the loss of a relative/friend due to COVID-19, anxiety related to the infection, and reliance on social media as a primary source of information on the pandemic. Vaccination significantly reduced the levels of fatigue among respondents. Our findings may add to the international debate regarding the long-term health consequences of the COVID-19 pandemic and strategies to manage them in the general population of South America.

Background: Paraguay has recently experienced an exponential increase in chikungunya cases, leading to psychological distress, particularly anxiety. Aim: To develop and validate the Chikungunya Anxiety Scale (CHIKAS). Materials and Methods: An initial scale of 18 items was used, which was subjected to validation by expert judgment to obtain 14 items. To determine construct validity, exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) have been employed, and internal and convergent validity were determined. Demographic, socioeconomic, and health status data were also collected. Results: The study included 974 participants. The final scale consisted of 12 items with evidence of a two-factor model (psychological and physical). The internal validity was good (McDonald's omega = 0.882). The CFA showed good adjustment indices. Regarding participant characteristics, a relationship was found between anxiety due to chikungunya and gender, employment, mental diagnosis, medication use, and chikungunya infection. Conclusion: The final 12-item CHIKAS had strong psychometric properties and was a two-factor model.

INTRODUCTION: The COVID-19 pandemic has led to an increase of social stressors and mental health issues in the general population as well as among mentally ill patients. A COVID-19- related "infodemic", including too much information in digital and physical environments, has been recognized globally. AIM: This study aimed to describe the impact of COVID-19 infodemic (exposure to news related to COVID-19) in terms of depressive symptoms in the Paraguayan general population. METHODS: This was a descriptive and cross-sectional study. An online survey, designed in Google Forms, has been launched nationwide through the most popular social networks (Facebook, Twitter, Instagram) and messaging applications (WhatsApp, Telegram) in April (1st-30th) 2021. The Mental Health Inventory-5 (MHI-5) was employed for detecting depressive symptoms. RESULTS: The survey included 1102 responders aged 35.4 ± 12.9 years old, 74.9% were women. MHI-5 mean score was 44.07 ± 14.16 in the general population with 34.4% (n = 379) of responders suffering from severe symptoms of depression (>52, as cut-off point). 53.5% of sample reported to have been exposed between 1 and 3 h to COVID-19 news, daily. An OR 1.933 (95% CI 1.48 - 2.52) was found between the exposure to news and depressive symptoms. CONCLUSIONS: This study suggests that people exposed to a higher number of hours of COVID-19 news were 93.3% more likely to develop depressive symptoms.

RESUMO

Atopic dermatitis (AD) is a chronic disease of the skin with significant pruritus and phases of exacerbation and remission. The hallmarks of AD are changes in the epithelial barrier and chronic inflammation. This condition is associated with an increased risk of developing mental disorders such as depression, anxiety, and suicidal ideation. It has been proposed that psychological stress may impact on immunological factors with an up-regulation of mediating neuropeptides at the level of central nervous system and secondary involvement of endocrine glands, peripheral nervous system, and immune resident skin cells. This article provides a concise review for dermatologists on the psychodermatology of AD, in order to highlight the putative common pathways between the dermatologic and mental health issues in this illness.

RESUMEN Introducción: los trastornos alimentarios y de la ingestión de alimentos están asociados con importantes efectos físicos y morbilidad psicosocial, así como con un impacto negativo en la calidad de vida de las personas. Objetivo: determinar la proporción de pacientes con riesgo de padecer algún trastorno alimentario y de la ingestión de alimentos en población general paraguaya. Metodología: estudio observacional, descriptivo de asociación cruzada, de corte transversal y temporalmente prospectivo. Una encuesta en línea fue difundida a nivel nacional a través de redes sociales (Facebook, Twitter, Instagram) y aplicaciones de mensajería (Whatsapp, Telegram). Se recabaron datos sociodemográficos y los participantes respondieron la Eating Disorder Examination Questionnaire (S-EDE-Q), en búsqueda de trastornos alimentarios. Se utilizó estadística descriptiva para todas las variables. Para buscar asociaciones se utilizaron la prueba t de Student y ANOVA, según sea apropiado. Para cuantificar los riesgos se utilizó Odds ratio. Resultados: participaron 375 personas, de las cuales el 70,9 % era del sexo femenino, con edad media de 31 ± 9 años. Se obtuvo una media de 2,09 ± 1,5 puntos en la Eating Disorder Examination Questionnaire (S-EDE-Q) y se encontró que 51 participantes tenían indicios de padecer algún tipo de trastorno alimentario. Conclusión: el 13,6 % de los participantes de esta investigación tiene indicios de padecer algún tipo de trastorno alimentario, las mujeres tienen 2,4 veces más chances de padecer un trastorno y existen una serie de factores que están asociados a un mayor riesgo en el desarrollo de algún tipo de trastorno alimentario.

Objective: to determine the proportion of patients at risk of suffering from a feeding and eating disorder in the general Paraguayan population. Methodology: observational, descriptive, cross-sectional study, of cross-association and temporally prospective. An online survey was disseminated nationwide through social media (Facebook, Twitter, Instagram) and messaging applications apps (WhatsApp, Telegram). Sociodemographic data were collected and the participants answered the Eating Disorder Examination Questionnaire (S-EDE-Q), in search of eating disorders. Descriptive statistics were used for all variables. Student's t test and ANOVA were used to look for associations, as appropriate. Odds ratio was used to quantify the risks. Results: 375 people participated, of which 70,9 % were female, with a mean age of 31 ± 9 years. A mean of 2,09 ± 1.5 points was obtained on the Eating Disorder Examination Questionnaire (S-EDE-Q) and it was found that 51 participants had signs of suffering from some type of eating disorder. Conclusion: 13,6 % of the participants in this research have signs of suffering from some type of eating disorder, women are 2.4 times more likely to suffer from an eating disorder and there are a series of factors that are associated with a greater risk for the development of some type of eating disorder.

RESUMO

There is currently an interest in evaluating the role that antidepressants may play in the treatment of primary cutaneous disorders. It has been proposed that antidepressants could have anti-inflammatory effects, but the clinical relevance of this effect has not been adequately established. In the case of bupropion, evidence for its specific use in dermatologic conditions currently come only from a pilot study and a case report. While this level of evidence is unlikely to be sufficient to guide clinical practice, the authors of this brief update hope to sort the available information to serve as a guide and provide a structure for future research.

Hair-pulling disorder (Trichotillomania) is a disabling mental disorder. Patient's behavior is characterized by the recurrent pulling of own hair with hair loss and a marked dysfunction in various areas of daily life. Trichotillomania is a relatively common disorder with pediatric onset, often associated with significant morbidity, comorbidity, and functional decline. Surprisingly, children or adolescents have been little studied in the research studies on the pathophysiology and psychopathology of trichotillomania. Furthermore, more evidences regarding the effective and evidence-based pharmacological interventions for the treatment of this condition are encouraged. This narrative review will report on the etiopathogenesis and clinical manifestations of trichotillomania including criteria for diagnosis and treatment issues of this complex mental disorder.

RESUMEN Introducción: el síndrome de Burnout es una enfermedad mental causada por el estrés laboral, situación ampliamente asociada con los médicos y otros profesionales de salud, asimismo, el abuso de sustancias y la depresión también son problemas psiquiátricos que están relacionados con este ámbito. Objetivo: identificar la frecuencia de estudiantes de medicina del área clínica de la Universidad Nacional de Asunción con sospecha de Síndrome de Burnout y trastorno de abuso de alcohol. Metodología: estudio observacional, descriptivo, de corte transversal en estudiantes de medicina del área clínica de la Universidad Nacional de Asunción de Paraguay. Se utilizaron encuestas con la prueba de identificación de desórdenes de uso del alcohol (AUDIT-C), el inventario de Maslach y el screening de depresión PHQ-2. Resultados: de las 157 encuestas, el 43,9% cumplió con los criterios de Síndrome de Burnout, el 49% cumplió los criterios de abuso/dependencia de alcohol y el 38,9% tuvo criterios de alta probabilidad de trastorno depresivo mayor, de ellos la mayoría presentó ideación suicida en los últimos 12 meses. Conclusión: los resultados fueron parecidos a los hallados en la literatura, sin embargo, ciertos aspectos fueron mayores en este estudio, como el porcentaje que cumple los criterios de Síndrome de Burnout, abuso de sustancias e ideación suicida. El Síndrome de Burnout y la dependencia al alcohol son frecuentes en estudiantes de medicina y se asocian comúnmente a trastornos depresivos.


ABSTRACT Introduction: Burnout Syndrome is a mental disorder caused by occupational stress, a situation widely prevalent in doctors and other health professionals. Substance abuse and depression are also psychiatric problems that are related to this area. Objective: To identify the frequency of Burnout Syndrome and its association with Alcohol misuse in medical students in their clinical years at the National University of Asunción. Methodology: Observational, descriptive, cross-sectional study in medical students from the clinical area of the National University of Asunción, Paraguay. Surveys were used with the alcohol use disorder identification test (AUDIT-C), the Maslach inventory and the PHQ-2 depression screening. Results: Of the 157 surveys, 43.9% met the criteria for Burnout Syndrome, 49% met the criteria for alcohol misuse / dependence and 38.9% had high probability crite-ria for major depressive disorder. Most of them presented suicidal ideation in the last 12 months. Conclusion: The results were similar to those found in the literature, however, certain aspects were greater in this study, such as the percentage that meets the criteria for Burnout Syndrome, alcohol misuse and suicidal ideation. Burnout Syndrome and alcohol misuse are frequent in medical students and are commonly associated with depressive disorders.

BACKGROUND: The management of mental disorders in child and adolescent is challenging. In addition, when substance use disorders are associated with mental disorders, important complications are noted. Dual pathology and Substance-induced disorders should be distinguished in cases like this. OBJECTIVE: To summarize the latest information on the epidemiology, etiology and management aspects of dual pathology in adolescents. METHODS: PubMed, Web of Science, PsycINFO and SciELO databases were searched to perform a narrative review. RESULTS: Dual pathology is the term used to designate the co-occurrence of an addictive disorder and at least one other mental health disorder, independently. To distinguish dual pathology from substance- induced disorders, the temporal association between the disorders in a longitudinal assessment should be emphasized. If the symptoms of the mental disorder appear prior to the substance use or after a long period of abstinence, a diagnosis of dual pathology should be considered. Patients with dual pathology present greater psychosocial issues, more medical problems, and worse prognosis than those with substance-induced disorders. The proposed etiological models agree that biological, psychological and social factors are shared in the development of these conditions. CONCLUSION: Healthcare systems should focus on creating policies that will allow early detection, preventive public health measures, and an integrated and coordinated care for these patients. Public health policies should create means to promote awareness and prevention of these pathologies since early initiation of treatment (pharmacological, psychotherapeutic, family therapy, education in schools, behavioral interventions and treatment of comorbidities) reduces the risks associated to substance use disorders and other negative consequences.

Reconocer los múltiples roles que tienen los docentes de medicina es importante para prepararlos para la compleja tarea de enseñar la ciencia médica. El objetivo de este trabajo fue determinar la importancia que dan los docentes de medicina de 3 universidades públicas de Paraguay a los "Doce roles de un docente de medicina" planteados por Harden y Crosby. Fue un estudio observacional, descriptivo, de corte transversal, con muestreo por conveniencia. Los participantes fueron docentes de Medicina, de la Universidad Nacional de Asunción, Nacional del Este y Nacional de Itapúa, con al menos 2 años de experiencia, que completaron un cuestionario que midió la importancia que dan a los doce roles del docente de medicina. Para ese fin, se utilizó una versión en español de la teoría de Harden y Crosby, traducida con autorización de sus autores. Participaron del estudio 73 docentes. La mayoría percibió su papel más importante como proveedor de información en las prácticas o en la clínica (4,68±0,57), como modelo de rol en el ámbito laboral (4,53±0,60) y en la enseñanza (4,45±0,67), y como planificador o participante en las evaluaciones del estudiante (4,41±0,62). Estos resultados nos permiten afirmar que el docente de medicina de Paraguay se ve a sí mismo como más que un instructor de clases teóricas o prácticas clínicas, sino cumpliendo muchos diferentes roles al mismo tiempo. Los docentes de medicina pueden desempeñar un papel importante en la realización de estudios de investigación pedagógica que permitan mejorar las prácticas de enseñanza y aprendizaje de la medicina(AU)


Recognizing the multiple roles that medical teachers have is important to prepare them for the complex task of teaching medical sciences. The objective of this study was to determine the importance that medicine teachers from three public universities in Paraguay give to the "Twelve roles of a medical teacher" proposed by Harden and Crosby. This was an observational descriptive cross-sectional study with convenience sampling. The participants were teachers of Medicine at the Universidad Nacional de Asunción, Universidad Nacional del Este and Universidad Nacional de Itapúa, with at least 2 years of experience, who completed a questionnaire that measured the importance they give to the twelve roles of the medical teacher. For that purpose, a Spanish version of the Harden and Crosby theory was used and translated with the authorization of its authors. Seventy three teachers participated in the study. The majority perceived its most important role as a provider of information in the clinical rounds or in the clinic (4.68 ± 0.57), as a role model in the workplace (4.53 ± 0.60) and in teaching (4.45 ± 0.67), and as a planner or participant in the student evaluations (4.41 ± 0.62). These results allow us to affirm that the medical teacher of Paraguay sees himself more as an instructor of theoretical classes or clinical practices, but they see themselves as fulfilling many different roles at the same time. Medical teachers can play an important role in conducting pedagogical research studies to improve the practices used for teaching and medicine(AU)
